The hepatic extraction of insulin (50%) and glucagon (5-28%) are quite different and independently regulated. Hepatic removal of insulin and glucagon plays an important role in both the homeostasis of these hormones and hepatic glucose metabolism. The regulation of these effects is incompletely understood but may be influenced by the amount of insulin in relationship to glucagon that reaches the liver. Although the hepatic glucose output is regulated by both insulin and glucagon levels, the effects of these hormones are not well understood. Hepatic glucose output correlated better with the portal venous insulin and glucagon molar ratio than the ratio of the hormones removed by the liver. The 75% of the total glucagon immunoreactivity represented 3,500 molwt glucagon are removed by the liver, while glucagon fractions, other than the 3,500 molwt fraction, are not removed by the liver.
