現在の関節リウマチ（RA）の治療目標は，臨床的寛解（罹病期間長期の場合は低疾患活動性），関節破壊進行の阻止（構造的寛解），ならびに身体機能の改善・保持（機能的寛解）である．

このうち，RA患者の身体機能を評価する方法としてHealth Assessment Questionnaire Disability Index（HAQ-DI: HAQ機能障害指数）1）が広く用いられている（表１）．

HAQ-DIを用いた身体機能に関する治療目標（機能的寛解）の基準は複数提示されているが，最も一般的に用いられているものは，OMERACT（the Outcome Measure in Rheumatology Clinical Trials）グループが2005年に提唱したRAの最小の疾患活動性（minimal disease activity）についての予備的な定義で記載されたHAQ-DIスコア≤ 0.5という基準2）で，メトトレキサート（MTX）未治療で罹病期間が2年以内の中等度から高度の疾患活動性を有するRA患者を対象として行われたCOMET（Combination of Methotrexate and Etanercept in Active Early Rheumatoid Arthritis）試験でもHAQ-DI ≤ 0.5をnormal HAQ-DIとして用いており，52週後のHAQ-DI ≤ 0.5到達割合はMTX単独群39％，MTX＋エタネルセプト併用群が55％と報告している3）．さらに厳格にHAQ-DI＝0を基準とするものもあり，MTXにアダリムマブを併用したRA患者を対象に有効性と安全性を評価したARMADA（The Anti-Tumor Necrosis Factor Research Study Program of the Monoclonal Antibody Adalimumab 〔D2E7〕 in Rheumatoid Arthritis）試験では，投与開始時から4年後にHAQ-DI＝0となった症例は22％であったと報告している4）．

　なお，HAQ-DIの改変版であるmHAQスコア5）は，HAQ-DIを簡素化した関節炎の機能評価のツールとして用いられることも多いが，HAQ-DIと比較してmHAQスコアは低値となる6）ため，これを用いて機能的寛解を0.5以下と定義することは不適切である．
